The Bit Foundation plans to expand the model to other health centres in the Balearic Islands.
Son Espases University Hospital participated in the International Congress on Health Outcomes (ICHOM 2024) presenting its innovative Health Tracker tool, developed in collaboration with the Balearic Foundation for Innovation and Technology (BIT Foundation). This event, which brings together world leaders in the measurement of health outcomes and healthcare quality management, was the ideal setting to showcase the hospital’s progress in improving healthcare quality through the use of advanced technology.

Health Tracker, an app developed by the Bit Foundation and Son Espases Hospital to collect data and improve patient care, is presented
Health Tracker is a digital platform specifically designed to collect and analyse data reported directly by patients (Patient-Reported Outcome Measures, PROMs). Through mobile-accessible questionnaires, patients can share key information about their quality of life, disease management and any complications related to their treatments. This information is automatically integrated into the Son Espases electronic health record (EHR), allowing healthcare professionals to access real-time data, facilitating more accurate and timely clinical decision-making.
Health Tracker is based on ICHOM’s Standard Sets to ensure standardised and focused measurement of the health aspects that matter most to the patient. The platform currently collects data on pathologies such as localised prostate cancer, cataracts, childbirth, psychosis, multiple sclerosis, stroke, chronic pain and low back pain, among others. In addition, work is underway to incorporate new pathologies, including migraine, Parkinson’s and dementia, to cover a broader spectrum of health needs.
Impact on healthcare and future projection
The implementation of Health Tracker at Hospital Universitario Son Espases has had a remarkable impact on healthcare, improving communication between patients and professionals and providing valuable information for treatment optimisation. This patient-centred approach supports more personalised healthcare aligned with the principles of value-based healthcare, where patient-reported outcomes play a key role in the evaluation of quality of care.
Thanks to the success of this project, the Bit Foundation plans to expand the model to other health centres in the Balearic Islands, consolidating its commitment to technological innovation in the service of public health. In addition, the ‘PROM Son Espases Model’ programme, which uses Health Tracker, has been recognised with the Professor Barea award for its outstanding contribution to the evaluation of results in the health sector, reaffirming the role of the BIT Foundation as a benchmark in digital health transformation.
